Account Self-Exclusion
We understand and support your decision to take a break from gambling.
If you would like to voluntarily self-exclude from all forms of gaming regulated by the New York State Gaming Commission now and in the future, for a period of 1, 3, 5 years or Lifetime, you will need to follow the procedure listed below:
You can do this by completing the form located here. After the form is completed, you will need to have it notarized and then mail the form, along with a minimum 2" x 2" original color photo of yourself and no larger than 4" x 6", which clearly shows your head and shoulders. A copy of a driver's license will not be accepted and photo must be recently taken within the past 6 months.
Please note: Your self-exclusion request will not be processed without proper notarization and a current photo. Final and notarized forms with photos may be submitted to address shown below:

Once you self-exclude, it will not be possible to reopen your account under any circumstances while you are self-excluded.
